The Basics

What Is a Medical Billing Clearinghouse, and Why Does It Matter?

A clearinghouse acts as the bridge between your practice and insurance payers. When your team submits a claim, it goes through the clearinghouse first — where it’s formatted, validated, and sent to the correct payer electronically. When the payer responds, the remittance comes back the same way.
The problem: Most practices are left to manage that relationship themselves. EDI setup, rejection resolution, clearinghouse portal access, remittance file mapping — it falls on whoever has time, which is usually no one.
Neolytix manages your clearinghouse relationship end-to-end. We handle the configuration, monitor the claim flow, resolve errors before they age, and map remittances back to your PM system — so your team doesn’t have to log into another portal.

What managed medical billing clearinghouse services include

Eight things, published as a list rather than described in prose — so it can be used to compare vendors, including against us.

EDI setup and payer enrollment

Payer enrollment, trading partner agreements and 837 configuration for your PM system. Enrollment status stays visible per payer and per provider, so nobody discovers mid-quarter that a payer was never enrolled.

Claim submission and format validation

Electronic batching, submission timing and format validation before transmission — so a file error is caught here rather than returned as a rejection days later.

Pre-submission scrubbing oversight

Claim edits and scrubbing rules monitored and updated, so errors are caught before the payer receives the claim rather than after.

Daily rejection management

Rejections tracked daily, root cause identified, and the correction resubmitted as soon as it is confirmed. Worked in filing-window order, not arrival order.

Electronic remittance and posting reconciliation

ERA retrieval, 835 mapping into your PM system, and posting reconciled against bank deposits rather than only against the remittance.

Eligibility verification support

Real-time eligibility tied to your scheduling workflow, so coverage problems surface before the claim exists.

Clearinghouse vendor liaison

We hold the clearinghouse relationship — configuration changes, outages, payer connection failures, support tickets. You never need portal access.

Rejection trend routed back upstream

The pattern goes to whoever owns the stage producing it — scheduling, registration, charge entry or enrollment. This is the part that reduces rejection volume rather than managing the queue better.

Items 07 and 08 are the ones to check with any other vendor. Most will monitor a queue. Far fewer will own the clearinghouse relationship, and almost none will send the rejection trend back to the team that caused it.
How Onboarding Works

How Clearinghouse Onboarding Works

Most practices are live within 2–3 weeks. Here’s the sequence.

Days 1–2

Practice Assessment

We review your current PM system, active clearinghouse connection, and claim workflow to identify any configuration gaps before we touch anything.

Days 3–7

Clearinghouse Configuration

We set up or migrate your EDI connection, configure payer-specific rules, and validate your 837 file format against each payer’s requirements.

Days 5–14 · concurrent

Payer Enrollment

We submit enrollment forms to each payer on your panel. Timeline varies by payer — we track and follow up on every one.
Days 10–16

Parallel Testing

We run test claims through the clearinghouse and validate end-to-end — from submission to ERA receipt — before go-live.

Days 17–21

Go-Live & Monitoring

Your claims move through the new configuration. We monitor submission reports and rejection queues daily for the first 30 days.
Ongoing

Ongoing Management

Daily rejection tracking, monthly reporting, and proactive rule updates as payers change their requirements.

Clearinghouse services by practice type and specialty

The eight items in scope are the same everywhere. What differs is payer panel size, enrollment volume, and how many practice management instances have to hold the same configuration.

Independent practices

Independent practices: one payer panel, no second person

One payer panel to maintain, but no second person to absorb portal time. The gain here is hours back at the front desk rather than a change in claim volume.

Multi-site groups

Multi-site groups: one configuration across every location

The same configuration held across every location, and one view of rejection rate by site — usually the first time anyone can see that one location generates most of the enrollment failures.

Behavioral health

Behavioral health: high claim count per patient

Session-based billing means high claim counts per patient, so a single format or authorisation error repeats at volume before anyone notices it.

Surgical & procedural specialties

Surgical and procedural: higher dollar value per claim

Higher dollar value per claim, which changes the arithmetic — one rejection sitting unworked costs more here than a week of them elsewhere.

PE-backed platforms & MSOs

PE-backed platforms and MSOs: one standard per acquisition

Newly acquired practices come onto one configuration standard rather than adding another process to manage, with reporting that rolls up across entities.

Enterprise & health systems

Enterprise and health systems: multi-entity, multi-PM environments

The same submission rules have to hold in each system, running alongside what already exists rather than requiring a migration.

Why Practices Choose Neolytix for Clearinghouse Management

We’ve been managing clearinghouse relationships for group practices since 2012. Here’s what that means in practice:
01

We work with your PM system, not around it

We support all major PM platforms — Athenahealth, eClinicalWorks, Kareo, Office Ally, and others. We configure the clearinghouse connection within your existing workflow, not a parallel one.

02

99.72% clean claim rate

Across our RCM client base, 99.72% of claims submit clean on the first pass. Rejection management is our backstop, not our primary workflow.
03

One team, not a handoff chain

Your clearinghouse management is handled by the same team running your billing. No vendor coordination, no finger-pointing when something breaks.

04

We manage the payer relationship

We have active EDI relationships with 1,800+ payers. For most practices, that means zero new payer enrollment paperwork on your end.

05

Enterprise-grade security and compliance

Your claim and remittance data moves through a clearinghouse and back — Neolytix is ISO 27001 certified and HIPAA compliant, and is pursuing HITRUST e1 certification. A Business Associate Agreement is available on request.
06

Built for scale

Neolytix supports 270+ organizations across 40 states and 31 specialties — including multi-location groups with complex, location-specific payer rules.

Denial Management Built Around Your Organization.

Built around your specialty, payer mix, and claim volume — not a standard package applied to every account.

FAQs

Medical billing clearinghouse FAQs

What is a medical billing clearinghouse?

The intermediary between your practice management system and every payer you bill. It validates claim format and payer-specific rules, transmits the claim in the standard the payer accepts, returns rejections, and delivers electronic remittance advice back. It is software with a payer network attached, not a service.

Is Neolytix a clearinghouse?

No. We manage your clearinghouse relationship — setup, enrollment, rejection work, remittance posting and vendor liaison — inside whichever clearinghouse you use or choose.

What is the difference between a clearinghouse rejection and a payer denial?

A rejection happens before the payer adjudicates: a format, enrollment or data problem stopping the claim at the door. A denial is an adjudication decision. Rejections do not appear in your denial rate, which is why a practice measuring only denials is understating the problem — and why aged rejections are the most commonly missed source of recoverable revenue.

We already have a clearinghouse. Can Neolytix still help?

Yes, and this is the more common arrangement. We take over configuration, enrollment and rejection work inside your existing clearinghouse. There is no requirement to migrate.

Do we have to change clearinghouses to work with Neolytix?

No. If a migration would genuinely help — poor payer coverage, chronic format problems, unworkable reporting — we will say so and manage it. Otherwise we work inside what you have.

How does clearinghouse setup work?

Assessment, configuration and parallel testing, with payer enrollment running alongside. Enrollment is the variable, because approval time sits with each payer — so we report enrollment status per payer rather than commit to a single date. Claims keep flowing through your existing configuration until the new one is validated.

What happens when the clearinghouse has an outage or a payer connection breaks?

We find out, we open the ticket, and we chase it. Your team hears what happened and when it is fixed, rather than discovering it through a drop in payments three weeks later.

Which clearinghouses does Neolytix work with?

The major national clearinghouses and most regional ones. The model is clearinghouse-agnostic by design — the work happens inside whichever platform you already use.

Which practice management and EHR systems do you work in?

The work happens inside the system you already run, so there is nothing to implement or migrate. athenaOne, eClinicalWorks, Epic, AdvancedMD and Tebra are among the most commonly encountered; others are supported.

Do you handle denials as well as rejections?

Rejections are in scope here. Payer denials — root-cause coding, appeals and multi-level escalation — are covered under denial management services.

Do you sign a Business Associate Agreement?

Yes, with every client, consistent with HIPAA requirements.
